This form is between Minki Hair Care ("Company") and the customer identified below ("Customer").

This Agreement documents the Customer's request to receive Minki Hair Oil in packaging that differs from the Company's standard packaging, and sets out the terms under which the Company will fulfill this request.

1. Product and Standard Packaging

The product covered by this Agreement is Minki Hair Oil. The Company's standard packaging for this product is a plastic container, which has been specifically selected for product safety, leak prevention, and shelf-life preservation.

2. Alternative Packaging

At the Customer's express request, the Company agrees to ship the above product in the following alternative packaging:

Alternative Packaging Description: Glass bottle, +/- 5% of normal quantity of 128mL.

This arrangement is a one-time accommodation made solely at the Customer's request and does not represent a change to the Company's standard packaging policy.

3. Customer Acknowledgements

By signing this Agreement, the Customer acknowledges and agrees to the following:

a)  The alternative packaging has been chosen by the Customer and differs from the Company's standard packaging.

b)  The Company makes no guarantees regarding the shelf life or product preservation of the alternative packaging.

c)  This Agreement does not affect the Customer's statutory rights in relation to product quality or safety.

4. Shipping & Liability

This shipment follows the Shipping & Returns policy, as with all other Minki Hair Care orders.

5. Agreement

By completing the form below, the Customer confirms they have read, understood, and agreed to all terms of this Agreement, and that their request for alternative packaging is made freely and voluntarily.
